Multiple Input Phoneme Therapy is an approach that is best suited for individuals with severe aphasia and/or severe apraxia. The best candidates produce very few utterances and are not yet able to consistently repeat a word after a model is provided. The goal of Multiple Input Phoneme Therapy is to develop the ability to repeat words, expand on any utterances the client already produces, and generalize the ability to produce utterances with intention (moving beyond stereotypical utterances).

For this therapy model, SLPs will collect a list of utterances that the client produces (gather input from friends, family, and caregivers). You will then work to break any perseveration by intentionally inputting the words (say one word at a time while the client listens) paired with a gestural cue (tapping the client’s arm). You will then gradually work on having the client repeat the word. Once the client has developed the ability to repeat and produce the word on cue, the next step is to leverage the speech sounds he or she is already producing. You will use phonemes, or sounds, from each word to create a new list of words to target. Then practice those words with the client to expand his or her vocabulary.
